Basic Needs and Wellness Liaison: Serve as the liaison between Catholic Charities of Steuben, Ira Davenport Hospital, and other medical providers to transform how Medicaid patients interface with health care. Assist vulnerable individuals and/or families with referrals to various Catholic Charities of Steuben services and programs and provide support, when able, to individuals beyond referrals through assessment of short-term emergency needs, advocacy with vendors, networking and collaboration with other human service providers, providing assistance in securing funding, and/or making referrals to other helping agencies. Collect and maintain client information through client database and track disbursement of funds through spreadsheet programs.
Peer Advocate: Function as a support for individuals utilizing Catholic Charities of Steuben Turning Point for basic needs and community navigation services.
Site Coordinator-Bath: Manage the day-to-day site based operations at Turning Point program in Bath, ensuring that policies and procedures are implemented and followed. Provide oversight to Turning Point staff, including IHS Transportation Specialist and any subsidized staff. Coordinate and implement Turning Point programs in the locality, creating a critical point of entry for people in economic crisis. Cultivate supportive interactive relationships with Catholic Charities programs, referring clients as needed. Assist Turning Point Assistant Director in monitoring contract compliance. Serves as primary coverage for all Bath site staff.
